Captain Churchill was a member of the 497th Tactical Fighter Squadron, Ubon Airfield, Thailand. On May 3, 1970, he was the weapons systems officer of a McDonnell Douglas Phantom II Fighter (F-4D) on an operational mission half way between Ban Ban and Nong Het in Xiangkhoang Province, Laos, when his aircraft was hit by hostile fire and crashed into a river. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
