Private First Class Clack was a member of the 1st Battalion, 35th Infantry Regiment, 25th Infantry Division. On January 1, 1969, he was crossing a river in South Vietnam, when he let go of the safety line was swept away by the water. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
