Major Coady was a member of the 602nd Special Operations Squadron, Nakhom Phenom, Royal Thai Air Force Base. On January 18, 1969, he was the pilot of a Douglas Attack Skyraider (A-1H) on a combat support mission 5 miles south-southwest of Tchepone, Savannakhet Province, Laos. While diving on his target, his aircraft was hit by hostile fire and crashed. His remains were recovered on February 10, 1994 and identified on November 28, 2001. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
