Lieutenant Commander Coakley, U.S. Naval Reserve, was a member of Attack Squadron 153, Carrier Air Wing 15 aboard the Aircraft Carrier USS CONSTELLATION (CVA-64). On September 13, 1966, he was the pilot of a Douglas Attack Aircraft Skyhawk (A-4C) on an armed reconnaissance over North Vietnam when a surface to air missile hit his aircraft and it crashed. His remains were recovered on December 15, 1988 and identified on May 23, 1989. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
