Specialist, Fourth Class Cudlike was a member of Company B, 1st Battalion, 506th Infantry Regiment, 101st Airborne Division. On May 18, 1969, he was fighting the enemy in Thua Tien Province, South Vietnam. He was wounded and being evacuated by a Bell Huey Utility Helicopter (UH-1B) which had to leave before he was fully aboard. He fell to his death from about 500 feet. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
