Major Lafayette was a member of Headquarters, Military Assistance Command, Vietnam. On April 6, 1966, he was the observer in a Grumman Mohawk Aircraft (OV-1A) on a visual reconnaissance over Laos, when it was hit by ground fire. He ejected safely but was surrounded by Viet Cong. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
