Notes
Colonel Levis was a member of the 42nd Tactical Fighter Squadron. On April 2, 1972, he was a crew member of a Douglas Destroyer Electronic Surveillance Aircraft (EB-66C) escorting a cell of B-52 bombers near the Demilitarized Zone, when his aircraft was hit by hostile fire and crashed. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
