Colonel Mitchell was a member of the 497th Tactical Fighter Squadron, Ubon Air Base, Thailand. On April 25, 1968, he was the pilot of a McDonnell Douglas Phantom II Fighter (F-4D) on a mission to find the surface to air missile sites in northern North Vietnam. After knocking out a Soviet aircraft, the aircraft was hit by hostile fire 5 miles northwest of Dong Ha, Quang Binh Province. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
