Command Master Sergeant Mullins was a member of the 4th Air Commando Squadron, Ubon Airfield, Thailand. On June 3, 1966, he was the flight engineer of a Chinook Spooky Attack Aircraft Gunship I (AC-47) on a mission 10 miles east of Ban Pha Philang near the borders of Savannakhet and Khammouane Provinces of Laos where his aircraft was shot down. His remains were recovered on June 20, 1995 and identified on April 28, 2003. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
