Captain O'Donnell was a member of the 433rd Tactical Fighter Squadron, Ubon Airfield, Thailand. On July 12, 1972, he was the weapons systems officer of a McDonnell Douglas Phantom II Fighter (F-4E) on a mission over North Vietnam, when contact was lost and the aircraft crashed into a reservoir.
