Chief Warrant Officer, Second Class Phelps was a member of the 299th Attack Helicopter Company, 7th Special Forces Group. On December 28, 1965, he was the pilot of a Bell Iroquois Utility Helicopter (UH-1D) departing An Khe, South Vietnam on a supply mission. Radio contact was lost. His remains were recovered on April 4, 2009 and identified on June 1, 2009. His name is permanently inscribed on the Courts of the Missing at the Honolulu Memorial.
