Captain Phelps was a member of the 4th Tactical Fighter Squadron, Danang Air Base. On November 23, 1971, he was the weapons systems operator of a McDonnell Douglas Phantom II Fighter (F-4E) on an operational operation over Laos. When he was about 20 miles northwest of Chavane, Saravane Province, Laos, his aircraft crashed and exploded. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
