Specialist, Fourth Class Phillips was a member of 114th Assault Helicopter Company. On September 26, 1966, he was flying in a Bell Huey Utility Helicopter (UH-1B) that was on an assault mission near the mouth of the Son Co Chien River in the Delta region in Vinh Binh Province, South Vietnam when one of the aircraft's rockets caught fire causing the aircraft to crash into the river killing him. His remains were recovered and identified on July 29, 2011. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
