Specialist, Sixth Class Shafer was a member of the 478th Aviation Company (Heavy Helicopter), 11th Aviation Group, 1st Cavalry Division. On April 19, 1968, he was the crew chief of a Skycrane Cargo Helicopter (CH-54) carrying a bulldozer into recently secured Landing Zone Tiger in Quang Tri Province, South Vietnam. The aircraft was hit and crashed. His remains were recovered on July 14, 1999 and identified on November 19, 2001. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
