Colonel Smith was a member of the 469th Tactical Fighter Squadron. On March 7, 1966, he was the pilot of a Thunderchief Fighter (F-105D) on a mission over North Vietnam, when his aircraft was shot down. His remains were recovered on November 6, 1998 and identified on April 17, 2000. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
