Specialist, Fourth Class Smith was a member of 117th Aviation Company, 214th Aviation Battalion, 12th Aviation Group. On October 3, 1968, he was the crew chief of a Bell Iroquois Utility Helicopter (UH-1H) on a command and control mission near Phouc Binh, Phouc Long Province, South Vietnam when his aircraft was hit by enemy fire, crashed and burned. His remains were recovered on November 30, 1994 and identified on June 1, 1999. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
