Lieutenant Colonel Smith was a member of the 23rd Tactical Air Support Squadron. On June 22, 1966, he was the pilot of a Cessna Bird Dog Observation Aircraft (O-1G) on a forward observer mission 5 miles southeast of Ban Muong Sen, Savannakhet Province Laos, where his aircraft was shot down. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
