Notes

Captain Thomas was a member of the 23rd Tactical Air Support Squadron. On July 6, 1971, he was the pilot of a North American Rockwell Bronco (OV10-A) on a visual reconnaissance 15 miles inside Laos west of Ben Het, when the aircraft disappeared. In 2014, the Department of Defense recovered a set of possible remains and through research, analysis and DNA testing an identification was made in 2017. His name is permanently inscribed on the Courts of the Missing at the Honolulu Memorial.
