Airman Apprentice Toms was a member of Attack Squadron 192, Carrier Air Wing 19 aboard the Aircraft Carrier USS BON HOMME RICHARD (CVA-31). On November 21, 1965, he was loading bombs on an aircraft on an elevator for an attack mission when the elevator moved causing him to fall into the sea. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
