Lieutenant Colonel Warren was a member of the 20th Tactical Reconnaissance Squadron. On December 5, 1966, he was the pilot of a McDonnell Voodoo Reconnaissance Fighter (RF-101C) on a mission over North Vietnam, when his aircraft crashed. His remains were recovered on September 17, 1986 and identified on December 11, 1986. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
